聚乳酸材料在亚洲患者下睑年轻化中的应用

Introduction

       Injectable fillers have been increasing in popularity for facial rejuvenation in recent years. One of the most challenging areas for filler injection is the lower eyelid. The area undergoes  many anatomic changes as the face ages.Many kinds of fillers and injection techniques  have been proposed, but the results are variable.4 We propose an alternative, simple, and  effective filler injection method in this area for Asian patients.

       近年来，填充注射技术在面部年轻化方面越来越受欢迎。填充注射最具挑战性的领域之一是下眼睑。随着面部年龄的增长，下眼睑区域发生了许多解剖学变化。以往研究提出了多种填充材料和注入技术，但结果各不相同。我们旨在探索为亚洲患者提出了一种简单有效的填充物注射方法。

METHODS

       Any patient who has tear trough deformities, infra-orbital hollows, nasojugal grooves, dark eye circles, as well as mild eyebags is a potential candidate for this method. Our filler choice is injectable poly-D,L-lactic acid (PDLLA; AestheFill; REGEN, Seoul, South Korea). PDLLA must be reconstituted with sterile water for injection (SWFI) before administration. When the patient's troughs, grooves, or volume deficit are class II to III, 3-4 mL of SWFI and 1 mL of  are used. When the deficits are class I, 5-7 mL of SWFI and 1 mL of  are used. With fanning injection technique, a wide region that covers all the deficits is evenly injected. The depth is under the dermis layer. Total amount of filler injection is not more than 2 mL on each side.

       本研究面向包含有泪沟畸形、眶下凹陷、鼻颊沟、黑眼圈以及轻度眼袋的患者。我们选择的填充物是可注射的聚D,L-乳酸（PDLLA; 韩国）。PDLLA在给药前必须用无菌注射用水（SWFI）进行复配。当病人的凹陷、沟槽或容积不足为II至III级时，使用3-4毫升SWFI和1毫升。当凹陷等级为I级时，使用5-7毫升的SWFI和1毫升的。采用扇形注射技术，均匀地注射覆盖所有容积缺失的广泛区域。其深度在真皮层之下。每侧填充物的总注射量不超过2毫升。

RESULTS

       Since 2019, we have injected more than 100 patients by following these steps in Taiwan. The level of patient satisfaction is high. Only some mild and transient post-injection complications such as edema, erythema, and ecchymosis were found.

       自2019年以来，本研究团队已经按照这些步骤注射了100多名患者。患者的满意程度很高。仅发现一些轻度和一过性的注射后并发症，如水肿、红斑和瘀斑。

CONCLUSION

       We propose an alternative, simple and effective method for non-surgical lower eyelid rejuvenation for Asian patients. However, anyone who wants to perform this procedure should be well-trained and knowledgeable about the anatomy, product and procedure to prevent the adverse events

       本研究为亚洲患者提出了一种替代性的、简单而有效的非手术下睑年轻化方法。然而，任何想进行这种手术的人都应该接受良好的培训，并对解剖学、产品和注射方法有所了解，以防止出现不良事件。
